What happened
Employee was installing data plates on the cylinders at the data plate station where he thought one was not correct he then left his station and went over by the unload lift to check the serial number he then stepped up onto the rollers to look at a serial number and data plate that was still on the line hanging and when doing so his left foot was near the corner of unload lift due to him stretching his left foot toward the unload lift because the line was moving the operator that was using the lift did not know he was so close and when he went to lower the lift it came down on the employees foot
