AE was kneeling down removing a fly-knife assembly from [REDACTED] 's A-side winder. Right hand was removing bolts while left hand was resting on idle roller above them (for balance)

When [REDACTED] stood up the idle roller moved and their hand struck an exposed blade that was near them. Blade was inside a guard but somehow the blade was exposed by about 1 16
