dumping scrap

[REDACTED] was dumping scrap out of a scrap hopper while using a forklift and the scrap was stacked above the top of the hopper. When the T[REDACTED] raised the hopper up to dump a piece of aluminum slid off the top of the hopper and went thru the mast striking the T[REDACTED] on the right mid forearm and required 10 stitches.
