[REDACTED] came in the office and stated that he had hurt his right hand while attempting to free up a crowbar that had gotten stuck inside the edge of the toolbox [REDACTED] tried to lean the box over against a bar on the mending frame to try and free up the crowbar as he did that the toolbox slid and associate tried to keep box from falling causing his hand to get smashed by the toolbox [REDACTED] had a cut on his right ring finger and cuts on his middle finger The middle finger looked like it had been smashed and the nail presented a small cut to the top part of the nail
