Performing routine cleaning of polish rolls.
What happened
Employee was performing routine cleaning of polish rolls only three safety blocks were being utilized one on the right corner between top and middle rolls and two on each corner of the bottom and middle rolls. Operator started walking from right to left between die head and polish rolls tripping over the chain causing him to fall forward and his right arm to go between the left side of the top and middle polish rolls. At this time the top left polish roll turn buckle bolt broke free causing the polish roll to fall on employee 's right elbow.
