What happened
On [REDACTED] at approximately [REDACTED] Supply Chain employee was beginning to load his 1st truck entering the graveyard shift 12a8a in which it was a curtain support beam van trailer The live load was spotted and set up during the previous shift The curtain van trailer had 2 support beams on each side the truck driver locked 1 in place and left the other free on both sides of curtain van trailer leaving the back end ready to load EE placed the dunnage onto the front of the trailer which is where the attached support beam was locked in place EE began to move the free support beam to the northside of trailer and then unlocked the other as he did he applied pressure and the support beam unlocked causing it to swing out striking EE in the nose EE failed to recognize that he was able to begin loading the back end of the trailer in which the support beams had already been removed and set up freely by the truck driver
