The ball mill had been jacked up using a bottle jack to remove one of the four support tires that had failed With the ball mill raised the bolts were removed from the pillow block bearing The injured employee then grabbed the left end of the shaft with his left arm in an attempt to lift the wheel assembly over the positioning blocks As he pull on the shaft he felt his left bicep pop at which point he let go The incident was immediately reported The wheel assembly weighs approximately 250 lbs
