The employee was loading pallets shells and carts onto the lift gate after completing their last stop. They were walking around the rear passenger side of the vehicle to step onto the liftgate and raise it up. They stepped over the support chain with their left foot and then while stepping their right foot over their toe caught the chain and they tripped themself. The employee spun and fell onto their right hip on the lift gate.
