Removing tire from chassis

Employee was working as a tire person at the Canopy with another since [REDACTED]. employee alleges that the while trying to remove the inner wheel on the front left side axle of a chassis he swung and hit the wheel with the tire hammer and hurt his left side of the torso with a pain that moved to his back. The employee went to the Paramedic 's office and later to my office to notify of the incident. While writing the report the employee complains of pain in his back and I took him to the paramedics to take him to the Hospital Industrial. As attachment the report from the Paramedic 's Office.
