As he was exiting the room there is a tall step-up. He could not see how high he needed to put his foot. His foot caught the edge.

He fell forward out of the door onto pavement. His left shin hit the edge of the cement step. He landed on his right knee and elbow. His glasses bent causing a scratch on his nose.
