Before the incident

What happened
Early on in the shift [REDACTED] picked up a piece of scrap metal out of the lower portion of the die. As he moved the piece to the scrap bin he misjudged the height of the bin and the scrap piece struck the lip of the bin. This caused the metal piece to drive backwards with the sharp point poking through his cut resistant glove and into his palm.
