What happened
The employee was working on line 88 to apply capsules by hand to the bottles as they moved to the foiler. She was standing on a small elevated platform to improve her height for application. The platform was covered with an ergonomic mat - that was larger than the platform.- When the employee went to remove the waste in her hand she moved her foot thanking she was stepping still on the platform but it wasnt and there was an elevation change. This elevation change made her off balance and the employee fell backwards first hitting her coccyx and then her head on the concrete floor.
