Associate was starting her job like normal and started with inserting the distal end of the coil into the distal collet She then pressed Start on the controller used to advance the travel carriage and started on the next step with inserting the Proximal end of the coil to the proximal collet This is where she ran into her problem with the controller not closing the proximal collet onto the proximal coil She then tried to bypass this step by pressing Start which some associates do to try to make the process shorter by pressing less buttons because it will usually close the collet automatically In this instance the proximal collet starts to Unwind the coil even though the proximal collet clamp arm didnt close on the coil and the edge of the collet holder was spinning on her pointer finger causing the laceration
