What happened
The worker was asked to setup an extension ladder for roof access. The worker was told not to exit the ladder to the roof. exposed weather barrier was slick from rainfall. The employee stepped on to the roof to reach something to secure the ladder. He felt his foot slip and chose to jump at the roof edge. Worker was at a height of 9 ft. he landed on his feet then tucked and rolled.
