Before the incident

As the employee was walking with a small bowl in her hand she rolled her left ankle and fell to the ground. Employee was not able to get back up on her own and requested ambulance. Ice was put on the ankle by an employee in the bakery department.

What happened

Employee rolled her left ankle as she was walking in the Bakery Department causing her to fall. employee complained of pain on the outer side of the left ankle. Employee 's son was called because the employee doesn 't speak or understand English. She speaks Hindi and Gujarati. Son 's name is [REDACTED] and his phone number is on this report for contact. Son stated that the injured employee has previously injured the same ankle in [REDACTED] when she slipped at another job and fractured it. he mentioned that the bruising on the inner side of the foot was caused by that injury as well as nerve damage. Employee was not wearing slip resistant shoes.
