Discharged patient [REDACTED] MR#[REDACTED] was verbally insulting Security Officer [REDACTED]. He was threatening to throw punches and kept using profanity to abuse the officer. I Security Officer [REDACTED] walked into the incident and started to receive threats as well. After asking Mr. [REDACTED] to leave the premises he removed his shirt and physically assaulted me.
