The employee was using a boom to work on a press. When work requiring the boom was completed the employee stored the boom on the lip of a hopper.

A supervisor noticed the boom was stored incorrectly and asked the employee to move the boom using a fork truck. The employee attempted to move the boom with the fork truck but when they reversed the boom fell and was stuck beneath the hopper. The employee got out of the forktruck and attempted to pull the boom free with their arm. When the employee freed the boom the pressure caused the boom the bounce up catching the employees arm between the boom and the hopper.
