Transloading freight

Driver was transloading freight with 2 other drivers. Driver then began to push a stack of 2 totes. He then pushed on the top tote and slid both across the floor. While doing this the bottom tote got stopped on a bump from the liftgate and the drivers momentum pushed the top tote off and then tripped on the bottom tote. This is when he fell and hit his face head on the tote.
