on [REDACTED] the client was escalating while i was on call trying to call the staff to come to pass a prn to the client he could calm down as i sitting close to the client he hit me on the mouth an act that was sudden swift that i could not evade. the following the client was crying as i asked what he wants he asked for his [REDACTED] which he struck with on my head temple. i decided to ER as i was in pain and dizzy.
