Before the incident
Operator ([REDACTED]) was attempting to remove film trim that had wrapped around an idler roller near the left side trim knife. Committing unsafe act.
What happened
Operator ([REDACTED]) thought he could remove the trim by hand. Attempted with left hand to reached up and pull trim out of machine while running at 700 FPM. At that machine speed the trim pulled his left hand back into machine and into contact with nip guard below upper idler roller.
