TM stated that while holding a bag with his good arm the bag slipped which required him to use both arms to catch the bag TM said he screamed due to the pain and the pain radiated up to his armpit TM informed Lead [REDACTED] and removed himself from work for the evening for his health and safety Please note that TM returned to work on [REDACTED] from previous injury on [REDACTED] and did not see his primary care physician as recommended by HF Health Occ TM was able to see his physician on [REDACTED] Claim to be investigated TM had appointment with HF Health Occ at [REDACTED] on [REDACTED]
