EMPLOYEE WAS FITTING A HEAD ON A TANK WHEN A WELD BROKE ON A LIFTING EYE FOR A RATCHET STRAP HE WAS USING TO FIT THE HEAD. THE UPPER WELD BROKE WHILE TIGHTENING THE LOWER RATCHET STRAP. THE UPPER RATCHET FELL STRIKING THE LEFT SHOULDER.
