Before the incident
working on loosening a bolt on a mold at his workstation around [REDACTED]. He had attempted to loosen the bolt with the biggest impact wrench he had with no success so he reverted to using a pull handle with cheater bar to help break the rusted bolt loose
What happened
As he was pulling on the cheater bar the bolt broke loose sending him flying backwards into the long edge of his toolbox. His toolbox wheels were not fully locked down so the toolbox rolled away and he continued to fall to the ground. He said it happened so quickly that he was unable to use his arms to catch himself in any way. So he struck his low back on the toolbox and then took the full impact of the fall to the same area of his low back.
