What happened
As he was pulling on the cheater bar the bolt broke loose sending him flying backwards into the long edge of his toolbox. His toolbox wheels were not fully locked down so the toolbox rolled away and he continued to fall to the ground. So he struck his low back on the toolbox and then took the full impact of the fall to the same area of his low back.
