Before the incident
IE was sucking dust for bottoms and there was a weighment not ready alarm indicating a clog jam. He proceeded to go to the top deck of the mixer mezzanine to repsond. It appears he removed the quick release fitting on the hose clamp then removed the vent filter stock. He used a telescope pole to clear any clog in the bottom of the weigh hopper. He inserted his arm through the vent on the weigh hopper and up into the rotary air lock coming in contact with the rotary air lock
