Using grinder in the die shop

Employee thought machine was off and went to clean the table contacting the grinder wheel and fingers pulled under grinder. All guards in place. Operator did not follow training and raise grinder wheel at least three inches above table surface. Grinder does not make any noise while running which is standard for that equipment.
