TM was on the forklift taking stacks off the rollers when the top pallet fell on to the TMs head.
What happened
TM was on the forklift taking stacks off the rollers. TM proceeded to pull the stack with his hands but couldn 't get them to move. TM went to the back side of the stack and started to push the stack. While doing so the top pallet fell on top of the TMs head.
