Before the incident
Employee was opening boxes of saline with a self retracting utility knife
What happened
The box of saline had two layers of tape and the blade of the utility knife was not sharp enough to easily penetrate the tape. Employee grabbed the box to allow the use of more force. The blade became stuck in the tape so the employee applied additionalforce to continue cutting the tape. The blade came detached from the tape rapidly which resulted in the blade striking her forearm.
