The employees statement is as follows: The time of the incident was [REDACTED] [REDACTED] and me was both driving double jacks I was driving double jack #30 or 31. Employee was driving in front of me Iwas behind her. She had 3 APC on her double jack taken them to Apps 2 side 2 when she was driving forward to place down the APC I was trying to blow my horn but my right side was not working but the left side was since I 'm so use driving right side I had no time to blow the other side she drove right into me causing the APC and my steering wheel to twist my lower body I had fall off my double jack and that 's when another employeel had came to check up on me and call the MBO I don 't remember her name for that night. I have two witness who saw everything [REDACTED] and [REDACTED].
