Before the incident
Loading a crated coil into an LTL trailer utilizing a sit-down Class 4 PIT.
What happened
Employee instructed by another Distribution Associate to load up an LTL trailer with 2 crated coils. As the employee was loading the LTL trailer the Distribution Associate who gave the instructions gave the driver the BOL to sign and a new seal. Driver stated that the Distribution Associate stated that the driver was good to go the Distribution Associate states they told the driver to wait 5 minutes. Employee instructed to load the LTL trailer grabbed the second crated coil failed to follow the Unscheduled Departure Audit and as the employee attempted to load the LTL trailer the driver pulled away from Dock Door #11 and employee in sit-down PIT fell out of the back of the trailer and off of the dock leveler resulting in the Employee sustaining a neck injury - Employee diagnosed with a cervical strain.
