What happened
A fall occurred during purging operations of the [REDACTED] skid when an employee encountered difficulty in opening the 6 in discharge valve due to the valve wheel being hard to turn and bracing obstructing access to the valve. To gain access IP approached from the opposite side and walked on the piping. While standing on top of piping the IP lost their footing and fell striking the 6 in header piping with the side of their head resulting in a momentary loss of consciousness and a contusion to the left side of their head.
