What happened

On [REDACTED] at approximately [REDACTED]. Supervisor of Times Square RCU was assisting PSE with the placement and scanning of the Amazon pallets parcels. I had placed one pallet of amazon parcels to the center of the scanning area after doing that I removed the jack and placed it under another pallet preparing it to be moved next. As I was doing this I did witness The employee cut the plastic off the centered pallet from the top. My back was turned as I was preparing the next pallet when I heard parcels fall and the employee scream. I rushed over to her and asked her was she okay 3 times as she was standing in front of a BMC shielding herself. All 3 times she did not answer. She later explained to me the parcels from the top of the pallet fell on her head neck and back. I offered the employee to sit down and take a break if she needed to take one. After 3 offers she sat down in the back and made a phone call to later request for forms [REDACTED]1 [REDACTED] AND [REDACTED]. I asked the
