PUTTING SEATBELT ON IN VEHICLE

At [REDACTED] while attempting to put seatbelt on leaned to right Carrier bumped head on door jam. Carrier was unable to continue after a break. No visible injuries to carrier. Carrier did not request medical but management felt it necessary to take carrier to medical since she was complaining of needing to throw up and headache. Carrier felt dizzy and ill.
