The operator leaned over the to view the position of the brush wheels from the top of the banded pack machine down to the magnetic conveyor belt. He leaned past the center of gravity as he stood on the ladder. The ladder slipped out from under his feet and the operator hit his shoulder on the yellow machine guard as he fell to the ground.
