EE had finished stacking flat metal parts on a pallet. He was in the process of wrapping the pallet and parts with plastic wrap. Another employee came over to help him and began spinning the turntable.

The other employee got the turntable spinning to fast and some of the flat parts fell off the pallet striking EE 's foot just above the steel toe cap
