Miner was in the process of removing a failed bolt from the roof of the UG. This is a common activity in the salt mine where roof bolts fail and need to be removed.

When the miner was attempting the pull out the loose roof bolt it came loose and fell catching the miners left hand between the rod and the edge of the metal platform they were standing on.
