At approximately [REDACTED] I received a call from a delivery clerk that a clerk was possibly injured. I arrived on the scene and the clerk was sitting in a chair in the clerks office. [REDACTED] stated that she was out of her truck in the [REDACTED] searching for a crate for delivery and stepped in an indentation in the ground causing her to roll her right ankle and fall. She had a scrape to her left knee as well. She was given an ice pack while being evaluated in the office. [REDACTED] asked for a doctors slip and an injury replacement was called at [REDACTED]. [REDACTED] was helped to her car and drove away on her own accord.
