Before the incident

The medium sized compressed nitrogen bottle that is used to control the end of pivot 2 ran out of pressure needing to be replaced. This low pressure causes the pivot to stop wherever it is. [REDACTED] took a replacement bottle as near to the end of the pivot as he could drive to but still had to carry the bottle some distance to the end tower of the pivot. There was approximately 6 inches of snow on the ground in the area and the ground was icy under the snow as well.

What happened

While carrying the nitrogen bottle without the valve cover in place he slipped on the ice under the snow. Typically the bottles are carried on the shoulder for safety and convenience. But [REDACTED] chose to carry it against his chest. When he slipped the valve head contacted his left cheek causing damage to 3 teeth in his mouth. Either the cheek contact with the valve or the employee biting down caused him to break a tooth off even with his gum-line. It is unclear from the employees description of the event if he is able to determine which of those are more likely causes of the tooth damage.
