Before the incident

Performing demo work on a conveyor with 3 other employees.

What happened

Employee was cutting a chain into two pieces using a burning torch. Said chain was approximately 48-52 inches off the ground. He was standing with a wide stance while standing back from the chain approximately 3 ft. in order to create distance from the work. When the chain finally separated it fell upon the typical coiled excess stack of chain already on the ground (approx 12 ' of chain total). When the free end of the chain landed on the coiled stack it caused the stack to fall sideways and come into contact with the top left metatarsal area of employee 's foot. These chains have prefabricated metal welded onto the sides of them so they can connect to the stainless slats of the conveyor. The prefabbed chain attachment had a sharp corner on it which struck the top of his foot when the stack fell over. He felt the impact of the chain land upon his foot and attempted to walk it off but then he realized his foot was punctured and bleeding.
