What happened
While the employee was installing a waler with the help of a Volvo Excavator with a bucket and hydraulic thumb attachment. The employee was measuring where the waler should be cut while an excavator operator had the beam clamped in between the thumb and bucket. The waler then became loose from the bucket and hydraulic thumb attachment falling to the ground and bouncing slightly then tipped over striking the employee on top of his left foot just behind the steel toe.
