What happened

While hoisting 10ft API rod with a continuous chain using the drill rig’s winch cable the top of the rod bumped against the drill mast while hanging at an angle and slipped through the chain rigging. The rod fell on top of the Chuck [REDACTED] 's left foot behind the safety toe portion of boot.
