Before the incident
The operator was troubleshooting a wiper machine alongside a technician. They were both trying to identify the issue when the technician walked around to the back of the machine to check the feeder bowl.
What happened
When the technician was by the feeder bowl the operator started to check the bottle inserts on the revolver. From where the technician stood near the feeder bowl the field of view to inside the machine was obstructed so the technician did not see the oeprator 's hand inside the machine. The technician then walked to the control panel which was in the out position and proeceeded to hit the revolver button. The revolver moved and caught the operators right index finger which was in the machine and trapped it against a pinch point below.
