Unloading butane boxes from container [REDACTED]
What happened
While unloading boxes of butane from a container van employee suddenly fell to the ground and sustained lacerations to his upper and lower lips. Our investigation revealed that he was standing at the opening of the container van while using one of the boxes as a step to reach the upper pile. While reaching for a higher box he overextended himself causing the box he was standing on to shift or tilt. This shift led to his loss of balance and accidental fall. As he fell his head struck a pile of unloaded butane boxes and the corner of one of the boxes caused lacerations to his lips.
