EE alleges [REDACTED] while taking out machines from the warehouse to the yard because there was a bunch of machines in the way and he wanted to clear the way and accommodate them he stepped off the forklift and tripped or his foot became stuck unknown on what and he fell on his knees and LOC and is unaware if he hit his head but states he injured both shoulders mainly the L shoulder. EE is unsure of what was injured because when he woke up he was in the ambulance. EE was taken to a hospital by EMS name and address unknown where he was given days off work but reports no follow up care instructions or work restrictions. EE reports last night he began to feel shoulder pain after getting home from the hospital and tingling in the knees. EE reports legs feel tired as if he did exercise and the L shoulder is swollen. EE also reports he feels like he has a fever and attributes it to the injuries. EE referred to the provider d t feeling as if he has a fever. EE reports he will go outside of r
