Before the incident
IP was inspecting the flow rates of the raw water tank as the rate in which raw water was flowing was not proficient for what was required.
What happened
While IP was inspecting flow rates for the raw water tank IP walked across a platform of multiple unsecured grating walkways. When IP walked back to egress the forward momentum of IP 's steps had pushed the grating backwards. While falling the grating moved sideways causing walkway to lose support. During the fall submersion into tank IP catches themself resulting in injury to left shoulder
