Before the incident
Employee was moving a large piece of plastic (by herself) and was kicking a pallet that was laying on the floor to adjust it to lay the plastic that she was holding on it. The other coworker had just stepped out to run for parts. She was to be doing a different task.
What happened
The employee was handling a large piece of plastic material and trying to adjust the pallet that she was going to lay the piece of plastic that she had in her hand on it by kicking it in place. She missed the pallet on the floor got her foot caught inside the pallet lost her balance and tried to catch herself on another pallet of material next to her and hit her right arm on the pallet of material and landed on the concrete.
