What happened
EE had just exited the main breakroom from a meeting the EE was heading easterly direction towards HR office on the [REDACTED]. As they passed Dock [REDACTED] 11 the dock plate at [REDACTED]11 had popped up from concrete flooring The EE did not see that the dock plate was elevated slightly as she continued to step she tripped on the elevated dock plate. This cause the EE to fall forward. As the EE began to fall they placed their hands out in front of them to brace for the fall. The impact jolted the EE (R) wrist. EE immediately notified their supervisor and was walked up to safety to be treated. EE was provided basic first aid and declined medical treatment.
