the associate was laying in the die working at trying to remove an insert out of the mounting block. Insert was located inside of the die space which required a vise grip to grab the insert.

while the associate was laying in the die trying to remove an insert when he clamped it with vise grips and was wiggling it back and forth. The vise grips popped off of the insert and his hand went in contact with the web of steel on the east side of the die.
