Team member was using a section of 1.5-inch round stock as a lifting device attached to a chain double bridle to release tension on a track tensioner.

As the crane was being raised to pull up on the track the round stock bent under the strain and the chain rigging came off the round stock and struck the team member in the face nose.

Broken nose and fractured upper jaw.
