Before the incident
TM was fixing the in feed at the front of the line where the forklift drivers load the line.
What happened
TM was standing in between the in feed rollers and the yellow guard rail. Another TM was driving the forklift and did not notice the bottom pallet was upside down and hanging. As the TM placed the pallets on the line the upside-down pallet caught the tip of the line and caused the other pallets to fall over the line and pinned the injured TM.
