Before the incident
TM was checking various machines to ensure that they were running correctly. TM was notified that the pusher #3 on the ADI repair was not working and went to check it out.
What happened
TM LOTO the line disconnected the power control solenoid and turned off the air supply. TM then reached out to turn the plate off the roller. When the TM stored air inside the cylinder caused it to retract pinching the TMs right arm between the cylinder plate and the guard. TM disconnected the air line from the cylinder.
